Community Based Habilitation Individual Services (CHIO)

Community Based Habilitation Individual Services provide community services to children with various needs.
We assist individuals with:

  • Leisure Activities
  • Daily Living Skills
  • Volunteer Opportunities
  • Hobbies

Based on your child’s needs, CHIO can be provided for as little as three hours per week to three hours per day. Services are provided on an individual basis in the community. Individual’s services are funded through the Bureau of Developmental Disability Services (BDDS) and are funded through the Family Supports Waiver and the Community Integrations and Habilitation Waiver. The phone number for BDDS is 574-232-1412. If you do not currently have a waiver for services for your child, please contact BDDS and they will assist you in completing the waiver application. Private pay services are also available.

Participant Assistance Care (PAC)

Participant Assistance Care is a non-goal based service provided to individuals receiving services on the Family Support Waiver. The Direct Support Professionals assist the person served with daily living skills in all areas.

Respite Care

Respite Care is a non-goal based service that gives primary care givers a break to complete activities that they need to, without having to worry about their loved one. Respite care cannot be provided while the primary caregiver is at work. Respite care can be provided in the community or at the child’s home. The Direct Support Professionals can sit with the child served and watch movies, help with meal preparation, medication reminders, or take the individual into the community to participate in activities of the child’s choice. Respite care can be provided in the community or at the child’s home.
